The day of the appointment, we recommend arriving to the clinic fifteen minutes before the scheduled time and not eating or drinking anything six hours before the procedure. If available, a copy of the dental insurance and a current (less than a year old) panoramic x-ray should be brought to us. We encourage a to bring a driver as a patient undergoing treatment under general anesthesia or conscious sedation cannot, by law, drive for at least 24 hours.
Walk-ins
We will always do the best to see a patient who comes into our office with a painful or urgent/emergent dental (which often may be medical) problem. We usually see our scheduled patients first, but attempt to accommodate patients in pain or with other serious problems as much as is possible.
Demographics
A demographics form must be completed before the first procedure with Columbus OMS. You can fill out the form online and print it or you can fill it out before the scheduled appointment when you arrive to our clinic.
If you're using Microsoft Windows, make sure that you have a PDF reader such as Adobe Reader installed. The completed form can be turned in our location before the procedure, or it can be faxed to (706) 507-0984.
